Consent Document for Storage of DNA Sample
Uses and Storage
By allowing Dirty Genes LLC, the provider of StrateGene, to store your DNA extracted from your saliva (your “Stored Sample”), you are consenting to having Dirty Genes and its service providers access and analyze your Stored Sample, using the same or more advanced technologies (such as genetic sequencing), in a manner consistent with our Terms of Service and Privacy Policy, including the Genetic Testing Privacy Notice. Unless we notify you otherwise, we will store your sample for a maximum of one year, at our third party laboratory. We may contact you in the event we need to re-analyze your Stored Sample. All of the same safeguards for any further use of your Stored Sample will be provided in our Terms of Service and Privacy Policy, including the Genetic Testing Privacy Notice.
Why your sample is stored: The College of American Pathologists (CAP) requires the storage period to be at least 1 year. CAP accredited ensures your test results are meeting and exceeding industry standards for clinical laboratory testing. Sample retention is a requirement by our third-party lab as retesting of the samples may be needed.
How it’s secured: The laboratory is inside a gated facility and monitored 24/7 by security. The laboratory itself is secured by fingerprint and ID authentication. The lab’s IT service provider provides encryption and weekly maintenance of access points.
